3. Disability ที่ครอบคลุม: Physical disability, Intellectual disability, Autism/ASD, Psychosocial disability (mental health), Sensory disability (ตาบอด, หูหนวก), Acquired brain injury → ต้องมี diagnosis จาก specialist + functional assessment
4. วิธีสมัคร NDIS: โทร 1800 800 110 (NDIS Contact Centre) → ขอ Access Request Form หรือดาวน์โหลดจาก ndis.gov.au → กรอกข้อมูล + แนบ medical evidence → ส่งทาง email, post, หรือผ่าน Local Area Coordinator (LAC)
5. หลักฐานที่ต้องใช้: (1) Diagnosis report จาก specialist (paediatrician, psychiatrist, neurologist), (2) Functional capacity assessment บอกว่าทำอะไรได้/ไม่ได้, (3) Reports จาก treating professionals (OT, speech, physio) → ยิ่ง evidence ละเอียดยิ่งได้ funding เหมาะสม

8. NDIS Plan Funding: แบ่งเป็น (1) Core Supports (daily living, transport) ใช้ได้ flexible, (2) Capacity Building (therapy, training) ใช้ตาม category, (3) Capital (equipment, home modifications) → Budget $15,000-$300,000+/ปี ขึ้นกับความต้องการ
9. ใช้ NDIS Plan: เลือก Plan Management (1) Self-managed = จัดการเอง จ่าย invoices, (2) Plan-managed = Plan Manager จัดการให้ (แนะนำ), (3) NDIA-managed = ใช้ได้เฉพาะ registered providers → หา providers ที่ ndis.gov.au/participants/finding-providers
